The latest study signifies an important rise in the number of young adults choosing permanent birth control methods such as tubal ligation and vasectomies following the Supreme Court's decision to overturn Roe v. Wade in 2022.

Abortion Restrictions Push Many To Choose Permanent Sterilization

Published in JAMA Health Forum, this research is the first to dig into the contraception decisions of people aged 18 to 30 post the Dobbs v. Jackson Women's Health Organization ruling, which limited the constitutional right to abortion.

Jacqueline Ellison, an assistant professor at the University of Pittsburgh School of Public Health and co-author of the study, emphasizes the significance of examining this demographic because they are more likely to undergo abortions and encounter upset concerning sterilization in comparison to older age groups.

While it's challenging to definitively attribute the increase in permanent birth control procedures to the Dobbs decision, the study employed a statistical method strongly suggesting a correlation between the Supreme Court's ruling and the surge in sterilization procedures, especially in states where abortion access faced further restrictions.

Analyzing medical records from academic medical centers and affiliated clinics before and after the Dobbs ruling, the researchers observed a notable uptick in tubal ligations and vasectomies post the decision.

The study acknowledges some limitations, such as the lack of state-specific data, which could provide deeper insights into the response to policy changes.

For instance, in Michigan, where the Dobbs decision allowed the enforcement of a 1931 law criminalizing all abortions, there was a temporary surge in requests for tubal sterilizations followed by a return to baseline levels after policy changes.

Additionally, the study solely focuses on patients who sought medical care during the study period, potentially overlooking those who did not seek such care.

Young Women Sensed a Heightened Need for Sterilization Procedures

Dr. Angela Liang, a clinical assistant professor of obstetrics and gynecology at the University of Michigan, suggests that young women may have felt a greater urgency to undergo sterilization procedures due to the disproportionate impact of pregnancy on them, including medical risks and post-pregnancy burdens.

Although a separate study by Megan Kavanaugh from the Guttmacher Institute found no significant changes in permanent contraception methods among a broader population of reproductive-age women in four states, Kavanaugh emphasizes the importance of awaiting data from the federally sponsored National Survey of Family Growth for a comprehensive understanding of contraception trends in the United States.

Dr. Kristen Mark, a professor of sexual health education at the University of Minnesota Medical School, noted the asymmetrical burden of pregnancy and parenting responsibilities, particularly on individuals with uteruses.

She explained that this asymmetry could drive them to take proactive steps, such as undergoing tubal ligation, to prevent unwanted pregnancies.

Dr. Elisabeth Gordon, a sexual health psychiatrist in New York City, highlighted the dual pressure faced by individuals capable of becoming pregnant-encouraged to undergo sterilization procedures while also facing resistance and questioning from medical professionals and societal norms.

While acknowledging the safety and commonality of vasectomies and tubal ligation, experts cautioned against making rushed decisions driven by external pressures or fears of unintended pregnancy.

Dr. Gordon stressed the importance of informed decision-making and supportive environments for individuals considering permanent contraception.

Despite the increase in these procedures, the focus remains on ensuring individuals have access to comprehensive contraceptive options and are empowered to make choices aligned with their needs and desires.
